How is qualifying the conclusion (D) different from providing support for a conclusion (B)?

To qualify something is to set limits on it; to put forth some of the conditions under which it holds true (e.g., "The Prime Minister later qualified his statements by saying he meant to refer to undocumented workers, not all immigrants")

I chose D instead of B.
However, is it correct to think that the combination of the first sentence (table salt fortified with iron) with the last sentence (people consume salt xxx) qualifies the conclusion, but not by only the last sentence?

No. The sentence in question does not place any limit on the conclusion, so (D) is incorrect. Instead, it offers us a reason to believe the conclusion is true (hence (B)).
