Test Day LSAT Scantron

Hey can anyone please help me with what to expect on test day in terms of filling out the scantron. Specifically, I want to know if we must memorize our LSAC account number, center number, and also what is the "Test Form Code," the "Test Form," and the "Test Book Serial Number" (#6, 12, and 13, respectively).
